Borderlands 4 contains 9 Vault Key Fragments (Vault Fragments). They are needed to unlock the 3 Primordial Vaults for the Nothing Left to Guard trophy & achievement.
Each main region has 3 Vault Fragments that open 1 Primordial Vault (The Fadefields, Carcadia Burn, Terminus Range). Dominion region doesn’t have Fragments or a Vault. You must find the 3 Vault Fragment collectibles in each region to obtain the Vault Key. Then the Primordial Vault icon appears on the map. When you approach it, a pillar will come out of the ground where you can insert the key. Then a purple portal appears, go through it to enter the Primordial Vault. There will be a side quest at the start of each vault. Accept it, then a yellow ramp activates. Use the ramp to be catapulted into an arena where you fight waves of enemies. You must fight through 3 arenas, then you encounter the Vault Guardian boss in the 4th arena. Defeating the boss gives you access to a permanent power-up. This is also a great way to farm XP.
Beating the Primordial Vaults at Ultimate Vault Hunter Rank 4 (unlocked after story) is needed to level up to Ultimate Vault Hunter Rank 5 (UVH5), which in turn unlocks the Gear Five trophy & achievement. After the story this can be tracked under Missions > Challenges > Ultimate Vault Hunter.
The Fadefields
Vault Fragment #1: Coastal Bonescape
Found at the southwest corner of Coastal Bonescape. At a house by the beach, on a box.
Vault Fragment #2: The Howl
Found in the middle of a moutain. Approach from the north of the icon and climb up the rocks. The vault fragment is halfway up the mountain on a plateau, not at the very top of the mountain.
Vault Fragment #3: Idolator’s Noose
Must pull a grate on the side of a building to enter a hidden room below the building.
Primordial Vault #1: Arch of Inceptus
Located in The Fadefields > Dissected Plateau. Only spawns after finding the 3 Vault Fragments in The Fadefields. When you approach a stone lever will come out of the ground where you can insert the Fadefields Vault Key. Then a purple portal appears, step through it to enter the Primordial Vault. On the inside you can accept a side quest, then fight through 3 arenas and beat the Vault Guardian boss at the end.
Carcadia Burn
Vault Fragment #4: Lopside
On the rooftop of a gas station on a dining table. Pull yourself up the grappling point at the front of the roof near the street.
Vault Fragment #5: Tonnage Peel
Found inside a tunnel. Drive halfway through the tunnel, there you find a grate on the floor that you can pull out. When you use ECHO-4’s scan functionality ( 
) it will be marked in green color to see it more easily. After pulling out the grate, drop down to reach the vault fragment.
Vault Fragment #6: Ruined Sumplands
Found on the side of some rocks, out in the open, easy to reach.
Primordial Vault #2: Arch of Radix
Located in Carcadia Burn > Grindstone of the Worthy. Only spawns after finding the 3 Vault Fragments in Carcadia Burn. When you approach a stone lever will come out of the ground where you can insert the Carcadia Burn Vault Key. Then a purple portal appears, step through it to enter the Primordial Vault. On the inside you can accept a side quest, then fight through 3 arenas and beat the Vault Guardian boss at the end.
Terminus Range
Vault Fragment #7: Cuspid Climb
This one is tricky to reach. It’s on top of a mountain that you must glide to. Start roughly 150 meters west of where the Vault Fragment icon is on the map, from the top of the cliffs. Then glide to the plateau that sticks out of the side of the mountain. There you can enter a tunnel to find the fragment at the end. See the red arrows in the image below for the path you must glide across.
Vault Fragment #8: The Low Leys
Found on top of a rocky path. You can jump up the many little rocks near the street level. At the top you must use a grappling point to pull yourself up.
Vault Fragment #9: Windspear
Inside an enemy base.
Primordial Vault #3: Arch of Origo
Located in Terminus Range > The Low Leys. Only spawns after finding the 3 Vault Fragments in Terminus Range. When you approach a stone lever will come out of the ground where you can insert the Terminus Range Vault Key. Then a purple portal appears, step through it to enter the Primordial Vault. On the inside you can accept a side quest, then fight through 3 arenas and beat the Vault Guardian boss at the end.
