Hell is Us includes 6 Vaults of Forbidden Knowledge. Opening one Vault is required for the trophy/achievement Vault Raider, but all must be opened eventually for items required for other trophies/achievements.
Vaults are locked hidden rooms that contain Relics, Glyphs, weapons, and lore items. You learn the solution for the door and a hint at the Vault location when turning in Research Items with Tania, but the solution isn’t random so you can go to the location and open the Vault at any time as long as you have access to the area it is in.
None of the Vaults are missable, so can all be opened after the story.

Vaults of Forbidden Knowledge
Vaults are listed in the order they appear in the Research – Vaults of Forbidden Knowledge menu. This is the same order that you receive the clues for them from Tania when turning in a certain amount of Research Items. You can tell if you have already opened in this menu if they display an opened door icon, but if you open a Vault before you gain the information about it from Tania it won’t appear in this menu.
#1 – Vault of the Copse: Senedra Forest
From the APC go west up the hill to the OMSIF camp you went to at the start of the game. Behind the tent is a cave you can walk into where you can find the vault door.
Open it by changing the dials to: Terror symbol, figure 8, Rage symbol, Circle with Spikes

#2 – Vault of the Knoll: Vyssa Hills
From the APC go east back into the main area, then follow the cliff to where you are above the waterfall near the stone ruins. You need to drop to the middle level of the waterfall from the upper ledge where you can find the vault door.
Turn the dials so that they are the following: figure 8, circle with 4 spikes, Ecstasy symbol, Rage, symbol

From the APC follow the path down toward the lake, then go southeast along the shore where you can find a cave, with the vault door at the back.

Go southwest from the APC toward the dig site, then down to the first level. Directly past the circular puzzle door you can find the vault door.

From the APC go to the southeast in the ruins area. Go around back past the ruins and into the cave with the tunnel leading to their center. In the tunnel you can find the vault door.
