Huge Hellbringers carrying the Blood Eye Portal on their backs have been witnessed trampling the lands of Sanctuary. Their existence was originally just a scary legend passed down in the tavern, a little joke used to scare tired travelers, but now it has been turned into reality by Mephisto's hatred. The locals call them aliens.
Planeswalkers can threaten entire areas at once, spawning nightmares created by hate from portals that fuse with their bodies. Planewalkers will spawn in an area at Mephisto's will, and after accumulating energy, they will begin to lumber around the area in search of holy sites. The alien walker at this stage cannot be harmed, but the terrifying Bloodbound Guardian will rush to protect the alien walker as it wanders around. Destroying the Bloodbound Guardian will eliminate part of the Outer World Walker's health.
When the Planewalker reaches the ritual location, it slams the ground and spawns three Hate Spires around it. Quickly dismantling these spiers will leave the Planewalker vulnerable before it can gather too much power.
